One of my favourite routes is the St Peter Ording line.

One question I have never been able to work out. Do you have to stop at the request stops. If yes, it is almost impossible to keep to the timeteble.

Do you get penalised for not stopping?

Zusi currently doesn't support simulating request stops, so yes, at the moment you need to stop at all of them.

Is it possible to get even close to the timetable when stopping at all the request stops?

I am OCD and maybe put too much emphasis on trying to get the perfect score!

There are already dozens of discussions regarding the score here, mostly in the german section of the forum.
Long story short, the score is not a good indicator about your driving style. In reality you could completely fail an exam even if you would get 100 % in Zusi and vice versa, perfectly pass an exam but get 0 % in Zusi.
In your case, keep in mind that safety goes before punctuality.

The score is a red herring. It should be possible to drive the train according to the timetable.

It is. The timetable doesn't require you to stop at every Request Stop - a real train which did would run likely run late as well.
